Job Summary
As a Provider Liaison, you’ll play a key role in growing program enrollment by building strong relationships with providers, referral sources, and community partners. You’ll lead outreach and public awareness efforts that support participant recruitment while ensuring all activities align with program guidelines.
This position is ideal for someone who is organized, self-motivated, and comfortable working independently while staying connected to a larger team. You’ll balance relationship-building with data tracking, planning, and follow-through in a fast-paced, mission-focused environment.
$63,000 Annually
ScheduleFirst shift, full-time hours. Specific work hours will be established in consultation with your supervisor.

- Develop and carry out outreach strategies to support participant recruitment
- Build and maintain strong relationships with providers and referral partners
- Represent the program in the community through meetings, events, and presentations
- Track daily outreach activity and key performance indicators
- Adjust strategies based on performance trends and enrollment goals
- Support advertising, public relations, and promotional efforts
- Assist with writing and distributing informational materials and news releases
- Ensure all outreach activities follow PACE and CMS marketing guidelines
- Coordinate events, presentations, and special outreach projects
- Maintain accurate data, reports, and planning systems
- Participate in quality improvement initiatives
- Support enrollment activities as assigned
- Complete other duties as requested

- Experience in marketing, outreach, public relations, or community engagement
- Experience working with older adults or willingness to complete training
- Must be able to move intermittently throughout the workday.
- Able to sit, stand, walk, bend, and lift as needed.
- Frequent local and regional travel required.
- Must be able to work under moderate pressure while meeting deadlines.
