In this regular part-time role, you will lead patient care initiatives at Chinook Reg Community Services. With a minimum of five years’ experience, you will be responsible for conducting assessments, planning care collaboratively, and managing transitions for patients and their families. Your expertise will be key in promoting health and providing support through various shifts across days, evenings, and weekends.

Key Responsibilities:

• Provide safe, quality, and family-centered care

• Lead comprehensive patient assessments and care planning

• Navigate complex discharge processes and advocate for patients

• Collaborate effectively in interdisciplinary care teams

• Manage difficult conversations with solution-focused strategies

Requirements:

• RN with active CRNA registration or eligibility

• Minimum five years nursing experience in good standing

• Basic Cardiac Life Support certification

• Skills in patient-centered care and concurrent planning

• Ability to handle high-stress situations and physical demands
