About the Position

Under the supervision of the Program Supervisor, the Lifespan Support Worker is responsible for intensive case management and systems navigation for individuals 16 years old and up who have or may have FASD, as well as for providing case management to caregivers who support such individuals.

Through coordination of a wide range of support services, the Lifespan Support Worker will strive to enhance the day‑to‑day living of the individual service participant by connecting them to services and supports throughout their lifespans, with the goal of limiting secondary disabilities and social issues commonly associated with FASD.

For caregivers, the Lifespan Support Worker provides overall case management, collaborates with community partners, strengthens existing natural support networks, and offers wrap‑around supports to assist caregivers with parenting and household management skills.

Key Responsibilities
  • Provide intensive case management and system navigation to individuals aged 16 and over who have or may have FASD.
  • Support caregivers of individuals with FASD, providing education, stability support, and advocacy.
  • Conduct case management with advocacy, crisis intervention, assessment, education about FASD, coaching, and supported referrals to community agencies, and develop individualized support plans.
  • Provide one‑to‑one support on daily living skills, advocacy, mentoring, supported referrals for community access, and crisis intervention services.
  • Maintain accurate file documentation, data collection, and case notes.
  • Collaborate with multidisciplinary team partners, other agencies, participant family members, and community, spiritual, or cultural resources.
  • Be flexible and able to travel to respond to program needs.
Schedule and Travel

The schedule is 8:30AM to 4:30PM, Monday to Thursday. Flexibility is frequently required to meet participant needs.

Requirements & Qualifications
  • Degree or Diploma in Human Services and membership in good standing with an applicable skilled association, or equivalent combinations of education and experience.
  • FASD education or training. Preference for those with prior experience supporting individuals with FASD and/or caregivers.
  • Knowledge and experience building relationships with community resources.
  • Understanding of diverse cultural identities, social issues, intergenerational trauma, mental health, addictions, and their impact on individuals with or who may have FASD.
  • Strong communication, advocacy, interpersonal, teaching, assessment, and intervention skills.
  • Proficiency with Microsoft Office (SharePoint, One Drive, Word, Excel, Outlook, etc.).
  • Current Standard First Aid CPR LevelC Certificate or CSA Standard Z1210‑17 Intermediate First Aid Certificate.
  • Clear Child Intervention Record Check dated within the past six months.
  • Clear Police Information Check with vulnerable sector, dated within past six months.
  • Valid Class5 Driver’s License, 3‑year non‑commercial Driver’s Abstract, and a reliable vehicle with $2M Third Party Liability insurance.
