OverviewAre you a compassionate and dedicated Registered Nurse looking to make a meaningful impact in patients’ lives? Join our dynamic home health care team as a Home Care RN and help clients receive skilled nursing care in the comfort of their homes, one patient at a time.
Responsibilities- Deliver exceptional nursing care to clients in their homes, ensuring highest standard of medical attention and support.
- Collaborate closely with a multidisciplinary team – therapists, social workers, and other healthcare professionals – to provide comprehensive, tailored care.
- Follow physician orders and work under the guidance and supervision of a Clinical Supervisor to ensure all medical protocols and care plans are meticulously followed.
- Build meaningful relationships with clients and families, offering medical care and emotional support throughout their health journey.
- Skilled Nursing Care and Comprehensive Assessment.
- Teaching, supervision, and counseling of clients and caregivers.
- Interdisciplinary collaboration.
- Problem solving and critical thinking skills.
- Monitor patient progress to ensure accomplishment of patient goals.
- Oasis documentation.
- Provide appropriate care for a range of physician‑ordered treatments (Pleur
X Drain, finger‑stick coagulation checks, wound care, wound vacs, IVs, phlebotomy, trachs, PEG tube, ostomies, telehealth).
- Minimum of one (1) year recent experience as a professional nurse in a clinical setting.
- Home Health experience preferred.
- Experience in hospital or acute care setting.
- Clear communication skills (both written and verbal).
- Ability to keep confidential information regarding patients and team members.
- Ability to withstand crisis situations.
- Customer service skills for patients, team members, and visitors.
- Proof of current/valid car insurance.
- Knowledge and experience with Electronic Health Records.
- Valid driver’s license with no more than 3 moving violations.
- RN – Registered Nurse.
- BLS – Basic Life Saving certification.
- Graduate of an accredited RN program.
Entry rate: USD $36.20/hr.
Maximum rate: USD $59.73/hr.
