Overview:

Are you an adaptable and skilled Registered Nurse ready to take on an exciting challenge? Join our innovative Acuity Adaptable Unit, where you will provide top-notch nursing care to patients with varying levels of acuity. This unique environment allows for a seamless transition of care, ensuring patients receive consistent and high-quality medical attention throughout their stay.

Responsibilities:

• Facilitates, implements, and evaluates the Patient Plan of Care and outcomes. Collaborates with the health care team to promote positive patient outcomes.

• Is seen as a patient advocate.

Skills:

demonstrates appropriate clinical skills and technical competence in providing patient care.

Critical Thinking: demonstrates appropriate clinical judgment. Assesses, analyzes and communicates patient information in a timely manner.

• Documentation:
Accurately and promptly documents all patient assessments, interventions , responses and provider communication.

• Assess patient safety risk factors and identifies those patients requiring specific safety interventions including skin integrity, fall risk & appropriate use of restraints. Follows medication safety policy. Promptly initiates appropriate interventions for safety needs. Completes Safety Tracking Tools as required.

• Delegation and supervision: supervises assigned staff, delegates appropriately, takes responsibility for all care provided to patients assigned. Reports all performance issues of team members to ensure all patients receive appropriate care and attention.

• Educational needs of patient:
Assesses educational needs of patient and family, provides appropriate written and verbal instructions. Documents assessment and education provided.

• Provider orders:
Performs ongoing review, initiation and implementation of patient orders in a timely manner every shift.

• Prioritization: prioritizes responsibilities and issues, taking into account urgency, patient needs, and seeks appropriate resources. Problem solves independently. Multitasks efficiently and demonstrates effective time management skills.
