We are seeking a dedicated Registered Nurse (RN) Case Manager to join our growing team in Lewiston, Idaho. As an RN Case Manager, you will play a critical role in coordinating and delivering high-quality home health and hospice services while building meaningful relationships with patients and their families.

• Complete comprehensive patient assessments and develop individualized care plans.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Provide skilled nursing care in accordance with physician orders and agency policies.
• Monitor patient progress and modify care plans as needed.
• Educate patients and family members regarding disease management, medications, and plan of care.
• Maintain accurate and timely clinical documentation.
• Participate in interdisciplinary team meetings and case conferences.
• Ensure compliance with federal, state, and agency regulations.

• Current Idaho RN license in good standing.
• Graduate of an accredited nursing program.
• One year of nursing experience required.
• Home health or hospice experience preferred.
• Strong assessment, communication, and organizational skills.
• Ability to work independently and effectively manage a caseload.
• Current driver's license, reliable transportation, and auto insurance.
