Position Summary:

Yellowstone Boys and Girls Ranch Targeted Case Managers manage all aspects of a youth’s case while providing critical community resources to support youth and families. Targeted Case Managers provide wrap‑around support for families while working collaboratively with current and prospective services to meet any youth’s complex needs.

Pay Range:

$20.38/hr – $22.94/hr

Responsibilities:
  • Advocate for youth and families in obtaining needed services, including assessing needs, referring to services, creating and implementing service plans, and monitoring services.
  • Maintain contact with other service providers to ensure the unique needs of the youth and family are best met.
  • Research community services to identify those that best meet the unique and individual needs of youth and families.
  • Collaborate with youth, families, and other professionals to develop a treatment/service plan that addresses the specific needs of the youth, including facilitating and leading treatment team meetings.
  • Complete, update, and maintain all necessary documentation within guidelines set by COA, Montana ARM’s, Contracts, and YBGR Policy and Procedures.
  • Complete, submit, and verify accuracy of paperwork, including progress notes, intake paperwork, etc., while maintaining PQI and financial viability of the program.
Required Qualifications:
  • 21 years old or older
  • Pass criminal record and child abuse registry background check required by the State of Montana.
  • Bachelor’s degree in a human services related field or equivalent experience.

  • Build rapport with youth and families therapeutically while serving as a positive role model.
  • Maintain professional therapeutic boundaries and confidentiality with youth and families.
  • Work with youth and families in multiple settings (school, home, community).
  • Work cooperatively with the treatment team.
  • Display reliable attendance at work.
  • Excellent active listening skills.
  • Complete forms within an electronic health record system typing at least 35 words per minute; familiarity with Microsoft Office 365 is a plus.
  • Demonstrate ethical judgment, decision making, self‑motivation, self‑discipline, and forward thinking.
  • Think of the team over self and define success collectively.
  • Effectively interact and communicate with others, building and maintaining relationships, collaborating, resolving conflicts, and navigating social situations.
  • Demonstrate the following core values:
