• Conduct comprehensive patient assessments and develop individualized care plans
• Administer medications, treatments, and skilled nursing procedures in home settings
• Monitor and evaluate patient health status, documenting changes and progress accurately
• Coordinate with physicians, therapists, and care team members to ensure continuity of care
• Educate patients and family members on disease management, medications, and home safety
• Maintain accurate clinical documentation in compliance with state and federal regulations
• Respond to patient needs and concerns with professionalism and empathy

• Active Registered Nurse (RN) license in the state of Maryland
• Current CPR/BLS certification required
• Minimum 1-2 years of nursing experience; home health experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license for home visits
• Compassionate, self-motivated, and able to work independently
