Bluegrass Care Navigators is seeking a Hospice RN Case Manager to deliver compassionate home-based care to patients and families in Fayette and Jessamine County. You will assess needs, build holistic plans of care, and coordinate with the interdisiplinary team to support comfort and dignity.
The role requires a valid Kentucky RN license, CPR certification, and reliable transportation.
Schedule:
Monday–Friday 8:00am–5:00pm; you will work primarily in patients' homes or local facilities.
