About Home Study and Post Release Services

KyUMH works in collaboration with the Office of Refugee Resettlement (ORR) to support children who are placed with caregivers across the state of Kentucky and potentially contiguous states. The goals of the Home-Study Post Release Services Program (HS/PRS) are to:

  • Assess the safety and suitability of a sponsor before an unaccompanied child is released into a sponsor’s care (Home Study).
  • Provide case management support to ensure the continued safety and stability of children in sponsor’s homes and linking children to resources in the community (Post Release Services).

The Intensive Case Manager is responsible for intensive case management services that the family needs additional support for. An Intensive Case Manager focuses on stabilizing the family unit and the intervention(s) utilized should be tailored to the needs of the family and connect the released child and sponsor family with community-based services to address concerns that require long-term care. PRS Intensive Case Managers provide Level Three (3) PRS and may also provide Level Two (2) PRS and Level One (1) PRS.

Required

Education and Experience
  • Master’s Degree in Social Work, Psychology, Sociology, or other relevant behavioral science in which direct clinical experience is a program requirement
  • Bachelor’s degree in one of the above categories with at least five (5) years of clinical experience
  • Highly proficient fluency in both English and Spanish, oral and written communication
  • Intensive Case Manager must have knowledge of local community social services and should have specialized experience working with immigrant populations
  • Intensive Case Manager should be knowledgeable about family preservation, kinship care and general child welfare
Preferred

Education and Experience
  • Current LPCA, LPCC, MSW, LCSW, LMFTA, LMFT, LMHC or eligible for licensure at the time of application
  • Experience working with diverse populations, including refugee and/or unaccompanied immigrant children and adolescents
  • One-year experience providing direct services to youth and families
  • One-year experience delivering community based and/or in-home services

Specific Duties and Responsibilities

The Caminos Post Release Services (PRS) Intensive Case Manager (ICM) provides community-based individual and family therapy to recently migrated children, adolescents, and their families.

The Intensive Case Manager is responsible for delivering culturally competent services that consist of mental health assessments, screening for human trafficking concerns, crisis intervention and support services, and psycho educational support groups for recently migrated youth as they adjust to life in the United States. The ICM supports the delivery of KYUMH’s trauma-responsive model by collaborating with the Post Release Services case management team to ensure the youth and their caregivers receive comprehensive services and are connected to both formal and informal resources in their community.

  • Conduct in-person visits and clinical meetings with clients in their homes on a weekly basis for at least the first 30 days of case assignment, and/or longer as needed based on individual clients’ psychosocial needs.
