We are hiring a Home Health Registered Nurse (RN) Case Manager for our Lexington office. • Salary Range $65,000 - $100,000 Sign on Bonus $4,000 3 days Fri, Sat, Sun or Sat, Sun, Mon with 9U productivity/Day

• Oversee the delivery of clinical and caregiver services, ensuring the highest quality of patient/client care in accordance with home health laws and regulatory standards.
• OASIS admissions and coordinate with a team of LPNs, CNAs, aides to ensure client and patient goals are met.
• Follow the Home Health orders from PCPs working closely with their office.
• Ensure medical supplies/equipment are delivered and staff are trained on care procedures.
• Assess patient/client needs, resolve issues, and ensure a high level of satisfaction.
• Effectively manage financial resources related to the delivery of quality patient/client care.

• Graduate of an accredited nursing program and active RN or BSN in Massachusetts.
• Minimum of 1 year of nursing experience in home healthcare/OASIS admissions, recerts, resumptions, and DC’s.
• At least 1 year of supervisory and caregiver management experience.
• CPR certification.
• Extensive knowledge of state and federal home health laws and regulatory standards.
• Excellent management, organizational, problem-solving, communication and interpersonal skills.
