Principle duties and responsibilities
Coordinates the care and discharge planning of hospitalized patients from a multi-disciplinary standpoint.Coordinates multi-disciplinary rounding to include: provider rounds, daily huddle, weekly discharge planning meeting, etc.May assist or conduct patient discharges by providing health education management to patients and their support systems, regarding post hospital care and may coordinate needed resources when applicable.Coordinates and collects quality data including call backs, population health data, swingbed and Utilization Review.Oversees reeducation in readmissions and over utilization of Emergency Room through chart reviews, process improvement, etc.Organizes and leads the Hospital Acquired Conditions meeting.Manages and oversees swingbed, intakes and referrals; complies with all swingbed rules and regulations, oversees completion of swingbed chart reviews. Communicates with multidisciplinary team to ensure patient centered holistic care and develop a multidisciplinary plan of care through Interdisciplinary Team meetings.Monitors patient care for quality and patient safety by completing chart reviews, quality management initiatives, etc.Manages and oversees Chronic Care Management program, while complying with all rules and regulations.Collaborates with the Director of Nursing and other leaders to develop, implement, and evaluate programs and services.Reviews patient and staff data to measure the effectiveness of patient care for staff evaluations.Practices responsibility with regard to budget to include staffing costs, operating costs, equipment and supplies.Organizes and leads the shared governance committee.Collects, reviews and reports for core measure data.Communicates all departmental concerns with the Director of Nursing and Chief Operating Officer.Maintains and updates policies to ensure evidence-based practice.Oversees Remote Utilization Review (UR), leads utilization review committee, and ensures UR chart reviews are completed.Participates in patient care activities that could include blood exposure and risk to blood borne pathogens (eg: lacerations, handling of blood-contaminated specimens, etc.Responsible for safe bagging and disposal of biohazardous waste.Responsible for following safe-injection practices.Responsible for ensuring the environment meets appropriate governing body standards.Ensure patients and visitors follow current infection control guidelines.Responsible for proper collection, storage and labeling of biological specimens.Maintains patient and staff safety through the use of patient safety tools (TeamSTEPPS, Just Culture, etc).Regular attendance at the assigned work location is required.Performs all other duties as assigned.Minimum knowledge, skills, and abilities
Knowledge and understanding of nursing theory and practice and the growth and development of a variety of patients in order to meet the patients' health care needs as is typically acquired through the completion of an Associate's or Bachelor's degree in Nursing from an accredited school. Bachelor's degree preferred.Must hold a current active Registered Nurse license from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act.BLS required within six months of hire and/or orientation.Two years nursing experience required.Ability to maintain strict confidentiality with regard to protected and sensitive information.Ability to complete reports and correspondence at a professional level.Ability to immediately respond to common inquires and complaints from patients, employees, and regulatory agencies.Advanced communication skills to respond to inquiries from Insurance companies and Workman Compensation companiesAbility to effectively communicate with individuals from diverse backgrounds.Successful completion of required knowledge and training of standard precaution and transmission protocols and when to apply during principle…
