Adoration Home Health and Hospice offers this private‑duty LPN position in Lexington, TN. The role is part‑time and focuses on 1:1 patient‑centered care.

Location: Brownsville, TN (Coverage area: Lexington, TN).

Schedule: Part‑time; every other Saturday/Sunday 6:30 am – 4:30 pm.

• Provide 1:1 care for client following the client’s plan of care, maintaining dignity and independence.
• Administer medications, feedings, oxygen, ostomy care as needed per plan of care and assessments of results.
• Accurately document nursing actions of all care given, communicate with family, case manager, and physicians, and document progress and outcomes for established goals; inform physicians, case managers, and families of changes in client’s medical condition and needs.
• Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are always on hand.
• Ensure all patient needs are met in accordance with physician instructions.

• LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• CPR professional certification.
• 1+ years of nursing experience preferred.
• Home Health care experience preferred.
