RN Nurse, Palliative Care Nurse, Healthcare Nursing
Company Description Connections Plus Healthcare + Hospice began in 1983 as a small, community-based effort and has grown to serve Lexington, Buena Vista, and Rockbridge County. Founded by four community members dedicated to providing compassionate end-of-life care, the organization has maintained a strong commitment to supporting patients and families. Its updated mission focuses on caring for neighbors living with serious illness so they and their families can enjoy life for as long as possible.
In 2023, Rockbridge Area Hospice and Mountain Regional Hospice united under the Connections Plus name, expanding reach and resources while retaining the same dedicated staff and high standard of care. The organization continues to invest in quality, community-focused healthcare and hospice services.
