As an integrated Trust, Lancashire and South Cumbria NHS Foundation Trust (LSCFT) delivers a comprehensive range of services, including:

• Primary and secondary mental health care for children and adults, including specialist inpatient child and adolescent mental health provision, perinatal mental health, and forensic services with low and medium secure care.
• Specialist mental health inpatient care for individuals with learning disabilities, alongside specialist community support for children and adults with learning disabilities and autism, including intensive support.
• Extensive community physical health and well-being services for children and adults, covering prevention, treatment, rehabilitation, and long‑term condition management.

The Trust was first established in 2002 and employs approximately 7,500 staff who provide care from more than 400 sites. Are you a patient-focused Registered Nurse looking for an exciting opportunity to develop your clinical triage and assessment skills within a new and evolving community service?

We are seeking a motivated and enthusiastic Band 6 Registered Nurse to work across the Urgent Community Response Team and Care Connexions. This role offers an excellent opportunity to be part of an innovative service supporting timely, joined‑up care for patients in the community, with a strong focus on hospital avoidance and ensuring people receive the right care, in the right place, at the right time.

The service currently operates across 7 days, 08:00hrs – 20:00hrs

This exciting new role will support the ongoing development of an integrated community response model, working in partnership with acute trusts, primary care, Northwest Ambulance Service, the voluntary and third sector, Social Care, therapy services and wider community teams

A key part of the role will involve clinical triage, ensuring referrals are responded to in a timely and effective way so that patients are directed to the most appropriate service. You will also undertake clinical assessments within the community and where required respond within the national agreed 2‑hour time frame, supporting timely intervention under the direction of Advanced Clinical Practitioners and working collaboratively with therapy colleagues to support safe, coordinated care planning.

We are looking for a Registered Nurse who is confident, compassionate and committed to delivering high‑quality community care. You will have excellent communication skills, a flexible approach and the ability to work effectively as part of a multidisciplinary team. Experience in community nursing and triage would be beneficial, although support will be provided to help you develop within this evolving role.
