BH Case Manager I – Western Kansas

Job Location: Field-based role. The clinician will meet members in facilities and/or homes across Western Kansas. Travel to client sites or designated locations is required, with occasional office attendance for meetings or training.

Geographic Eligibility: Kansas licensed masters level clinicians residing in Dodge City, Garden City, Liberal, Hays, Scott City, Norton, Phillipsburg, Smith Center, St Francis, Goodland, Ulysses, Lakin, Ness City, Hutchinson, Pratt, Hugoton, Great Bend, Marysville, Junction City, Manhattan.

Key Responsibilities:

• Uses appropriate screening criteria knowledge and clinical judgment to assess member needs.
• Conducts assessments to identify individual needs and develops care plan to address objectives and goals identified during assessment.
• Monitors and evaluates effectiveness of care plan and modifies plan as needed.
• Supports member access to appropriate quality and cost‑effective care.
• Coordinates with internal and external resources to meet identified needs of the members and collaborates with providers.
• Provides case management services telephonically and/or by home visits within the scope of licensure.

Position Requirements:

• Degree: MA/MS in social work, counseling, or a related behavioral health field, or a nursing degree.
• Minimum of 3 years clinical experience in social work counseling with a broad range of experience in complex psychiatric and substance abuse treatment; or an equivalent combination of education and experience.
• Current active unrestricted license in Kansas (such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist) that allows practice within the scope of licensure.

Preferred Qualifications, Skills, and Experiences:

• Experience in case management and telephonic and/or in‑person coaching with members having complex psychiatric/substance abuse and/or medical disorders.
• LDAC LAMFT (where applicable) for government business is also acceptable.
