Outpatient Licensed Practical Nurse – LPN LVN
Purpose and Scope: The LVN PPS provides direct quality care and treatment to patients requiring PPS-related services. Promotes patients’ physical comfort and provides emotional support to the patient and family.
Principal Duties and Responsibilities- Respond to all calls in a professional manner; take accurate messages and ensure that messages are delivered to the appropriate person. Document telephone contacts in accordance with policy and procedures.
- Facilitate communication and team involvement among providers, other staff, patients, and other service partners through timely sharing of information.
- Facilitate provider assessment by gathering information according to established policies & procedures. Preview all M.R. charts prior to the patients’ appointments, ensuring all pertinent records are in the chart.
- Monitor patient status during specialized procedures performed by physician/provider and gather data pertaining to patients’ condition prior to, during, and after the procedure (e.g., TMST).
- Legibly document appropriate patient care information in the M.R. chart in a timely manner and maintain the medical record chart appropriately.
- Perform direct patient care within scope of practice; cross‑cover nursing as required.
- Administer medication, treatment, and tests as ordered, in accordance with applicable state statutes, regulations, and policies & procedures.
- Refer telephone messages to appropriate personnel and document telephone contacts per policies & procedures.
- Conduct phone triage for patients with CP/SOB, documenting legibly and accurately.
- Adhere to all policies and procedures relative to the overall organization, including safety regulations concerning equipment, facilities, and hazardous materials.
- Act as a patient advocate in the coordination and delivery of quality patient care; consistently meet patients’ expectations for service and provide comfort and safety during the visit.
- Complete patient teaching using established protocols as required.
- Maintain a clean & safe environment and participate in overall quality management activities.
- Respect the rights, privacy, and confidentiality of patients at all times, remaining courteous, helpful, and professional.
- Maintain optimum safety standards for all patients.
- Pursue professional development opportunities and contribute to staff development and training.
- Review and comply with the Code of Business Conduct and all applicable company policies and procedures, as well as local, state, and federal laws and regulations.
- Assist with various projects as assigned by direct supervisor.
- Other duties as assigned.

- Frequent (34‑66% of the day): sitting, standing, walking, reaching with hands and arms, flexing/extension of wrist use, grasping and fine manipulation, talking and hearing.

- Infrequent (1‑11% of the day): crawling or crouching.
- Must be able to lift, pull, and push up to 25 pounds.
- High School Diploma required.
- Must possess current LVN license.
- 2 – 4 years’ related experience.

- Proficient with PCs and Microsoft Office applications.
