- Perform psychosocial assessments, considering family, social, and medical history; maintain up-to-date knowledge of medical and mental health diagnoses; report suspected abuse and neglect to mandated authorities; complete documentation accurately.
- Collaborate with the health care team to develop treatment plans and identify appropriate resources; provide real-time accurate information to patients/families/caregivers on eligibility and available resources; evaluate effectiveness of interventions and adjust as needed.
- Provide psychosocial services per the patient’s comprehensive plan of care; offer supportive counseling to improve emotional coping and adherence; educate patients/families/health care team on treatment options; give self-management support to high-risk/complex patients; conduct groups/programs for special populations; give anticipatory guidance; facilitate family-centered care team meetings; provide crisis assessment and interventions.
- Engage in interdisciplinary collaboration to improve the system, advocate for patient/family needs, mediate within the medical team, empower families in decision-making, and minimize fragmentation of services.
- Partner with patients, families, and the health care team for smooth transitions to next care levels; educate health professionals or students on psychosocial aspects of patient care; promote positive work environment; participate in professional development and attend meetings and conferences.
- Master's degree in Social Work
- Minimum of 2 years of work experience in a related discipline
- Valid LSW license; additional clinical license, certification, or training may be required
