Location and PayNewton Falls, OH – $26.00–$28.00 hourly.

- Implement the medical plan of treatment and follow physicians’ orders.
- Monitor individuals at all times, following all CHC and agency safety protocols.
- Transport clients to activities, services, appointments, and general community per ISP, HAH, and agency guidelines.
- Provide skilled care (G‑Tube, meds, etc.) to individuals based on the care plan.
- Document activities and services accurately and timely according to CHC and agency regulations.
- Monitor general health conditions of individuals and notify the supervisor of any medical issues or required treatment.

- Ability to work in a professional, ethical, and respectful manner.
- Ability to provide personal care for non‑ambulatory clients, use a Hoyer lift, maneuver wheelchairs, and transport clients to community events and social activities.
- Strong communication skills, oral and written; strong interpersonal skills.
- Ability to anticipate and resolve problems, demonstrating good judgment.
- Current LPN license in the state of employment.
- Current CPR/First Aid certification.
- Current TB screening.
- Minimum one (1) year of experience providing skilled care to medically fragile patients.
- Regular travel on a daily or weekly basis required.
Responsible for the delivery of direct, hands‑on skilled care of clients in homes or facilities. Responsible for assessing clients, documenting observation and assessment data, evaluating outcomes, and implementing the nursing plan of care.
