RN Case Manager

RN Case Manager – Liberty, MO

Hours:

M-F 8:30am-5pm

RN Case Manager will be responsible for assisting the team with on‑call, case management, visits, and coverage during call‑outs. Job duties can include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs
• Regularly re‑evaluating the patients' nursing needs
• Using assessment data to determine care plan needs
• Collaborating with the entire healthcare team, patient and family, to provide continuous and well‑coordinated care
• Counseling the patient and family in meeting end‑of‑life needs
• Initiating appropriate preventative and palliative nursing procedures
• Communicating with the physician regarding patient’s needs and reporting any changes in condition
• Supervising the hospice aides

Education:

Associate’s Degree in Nursing

Professional

Experience:

• State‑certified RN license
• At least 2 years of professional nursing experience (hospice preferred)
• Valid driver’s license and reliable transportation
• Willingness to travel
• Strong verbal and written communication skills
