Liberty Health Care Center is seeking a dependable, compassionate Licensed Vocational Nurse (LVN) to join our skilled nursing team. Your Responsibilities Shift Leadership & Coordination
  • Receive and give thorough shift reports; review MAR/TARs, new orders, and safety alerts.
  • Prioritize assignments, support CNAs, and ensure care is delivered efficiently and safely.
  • Perform scheduled and PRN rounds; maintain readiness for new admissions.
Medication & Treatment Administration
  • Complete med passes accurately and on time; follow the Five Rights of medication administration.
  • Administer treatments and manage PRN medications per provider orders.
  • Handle controlled substance counts, reorders, and documentation per policy.
Clinical & Skilled Nursing Duties
  • Provide wound, respiratory, urinary, and enteral care within scope and training.
  • Perform IV therapy if certified; obtain specimens and monitor vital signs and condition changes.
  • Notify providers and families of significant updates; document timely and thoroughly.
Resident Experience & Teamwork
  • Partner with the DON/ADON/RN Supervisor and interdisciplinary team members to meet resident goals.
  • Communicate clearly, compassionately, and professionally with residents and families.
  • Step in to assist with ADLs as needed — teamwork always comes first.
Compliance & Safety
  • Maintain EMR documentation accuracy and ensure survey readiness.
  • Follow infection control protocols, complete required logs, and uphold resident dignity and confidentiality.
Qualifications
  • Active, unencumbered Texas LVN license (required).
  • CPR/BLS certification (required); IV Therapy certification preferred.
  • Knowledge of SNF/LTC regulations, standards, and documentation.
  • Strong clinical judgment and leadership abilities.
  • EMR proficiency (Point Click Care  preferred).
  • Physical ability to lift 60–70 lbs, stand/walk for long periods, and assist residents safely.
  • Reliable attendance, positive attitude, and teamwork mindset.
  • Must meet all pre‑employment requirements (background, drug screen, TB, health clearances).
