We are seeking a compassionate and dedicated Registered Nurse (Home Health) to join our client’s growing team in Libertyville, Illinois. In this role, you will provide high-quality, patient-centered nursing care to individuals in their homes, helping them achieve and maintain optimal health and independence. The ideal candidate will be clinically skilled, self-motivated, and passionate about delivering personalized care in a home setting.

• Perform comprehensive patient assessments and deliver skilled nursing care in home environments
• Develop, implement, and adjust individualized care plans in coordination with physicians and the interdisciplinary care team
• Educate patients and families on disease processes, medications, and post-care instructions
• Monitor, document, and report changes in patient condition to ensure continuity of care
• Ensure timely and accurate clinical documentation in compliance with agency and regulatory standards
• Coordinate care with healthcare providers to promote positive patient outcomes

• Monday through Friday, day shift
• One weekend per month
• Two to three on-call days per month
• Two holidays per year
• This is a Pay Per Visit position

• Local travel required within the Libertyville area (travel reimbursement provided)

• Associate degree or higher in Nursing
• Active Registered Nurse (RN) license in Illinois
• Minimum of 1 year of professional RN experience (home health experience preferred)
• Current BLS or ACLS certification through the American Heart Association
• Valid driver’s license, reliable vehicle, and current auto insurance

• Prior experience in home health or community-based care
• Strong clinical assessment, organizational, and communication skills
