We are seeking a compassionate and skilled Registered Nurse to join Com For Care  Home Care as an RN Case Manager. You will play a vital role in helping clients live independently and enjoy their daily lives.

• Become a Health Detective: Conduct in-depth assessments to uncover each client’s unique needs.
• Master Planner: Use clinical expertise to create personalized care plans focused on quality of life.
• Make a Real Impact: See the direct results of your work as clients achieve medication safety and independence.
• Collaborate with the Team: Work closely with caregivers and other professionals to build a supportive network for each client.

• Graduate of an accredited nursing program.
• Illinois Registered Nurse (RN) license.
• Minimum one year of RN experience.
• CPR certification and strong knowledge of safety and infection control.
