Overview
The Clinical Lead for Community Services reports to the Head of Clinical Services and is responsible for overseeing and ensuring the delivery of high-quality, compassionate care to patients receiving palliative care in their homes and community settings. The Clinical Lead coordinates care services, manages clinical governance, and drives service improvements to enhance patient outcomes. The Clinical Lead leads the Community team operationally and is responsible for the service within the community.
You will operationally manage the Community Service team and support the community team in reviewing, assessing and treating patients within the community with complex needs. The Clinical Lead for Community Services will not hold a caseload.
St Giles Hospice is a registered charity founded in 1983 by the Reverend Paul Brothwell, originally to improve the care of local people dying with cancer. We now support patients, and their loved ones, living with a terminal illness. Our dedicated team provides individually-tailored care, free of charge, either at the hospice or in patients own homes across our communities. We spend close to £10 million a year providing our specialist services.
With just 18% of this funded by the Government, we rely heavily on donations and income generation from the local community.
