Hospice RN & Care Coordinator - Compassionate End

Gentiva is seeking an experienced Hospice RN / RN Case Manager to guide patients and families through the hospice journey, delivering hands-on nursing care in patients' homes or alternative settings. The role emphasizes interdisciplinary collaboration, education, and compassionate support for patients and their loved ones.

You will perform assessments, document care, and coordinate plans with the IDG, ensuring compliant, timely, and patient‑centered care.
