Overview
Lincoln Park Healthcare Campus is seeking a Full Time Social Worker position.
Qualifications- Graduate of an accredited school in social work or related field; having a bachelor’s degree in social work or a bachelor’s degree in a human service field including but not limited to sociology, special education, rehabilitation counseling, and psychology.
- State certified by the Board of Social Work Examiners as CSW or LSW.
- Experience in Subacute and Long Term Care.
Performs social services based on the facility’s policies and procedures as regulated by the State and Federal laws. PERFORMANCE EVALUATION: A competency based performance evaluation has been made part of this job description. (25 hours)
Job Responsibilities- Accurately conducts social service assessments and interventions and documents accordingly within the facility’s standards based on the State, Federal regulations & Joint Commission standards.
