Position: Hospice Registered Nurse (RN)

Suncrest Hospice is changing the expectations of health care with our exceptional clinical care. We are looking for a Registered Nurse to care for our patients. The Registered Nurse Case Manager works Monday - Friday, daytime hours and is available to candidates interested in full‑time opportunities.

  • Pay Scale $90,000‑$97,500 (commensurate with experience)
Responsibilities
  • Provide care and services for a group of patients, visiting patients in private homes, Skilled Nursing Facilities, and assisted living facilities.
  • Contribute to the interdisciplinary team, including psych‑social assessments, interventions, and knowledge of community resources.
  • Provide 1‑to‑1 patient care and collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Perform independent, day‑to‑day activities: phone, email, written communication, meetings with healthcare professionals, and ensure compliance with hospice rules and regulations.
  • Manage time: schedule visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns.
  • Learn an electronic medical records system.
  • Travel across the service area using reliable transportation with a valid driver’s license and auto insurance.
Qualifications
  • Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
  • Excellent teamwork and collaboration skills with nursing and support staff.
  • Strong communication skills with patients, families, and healthcare professionals.
  • Ability to work independently and manage activities such as phone, email, and electronic documentation.
  • Valid driver’s license and reliable transportation.
  • Ability to pass a background check and drug screening.
