Base pay range: $ / yr – $ / yr. Competitive compensation includes an annual bonus up to $10,000 and a 401(k) plan with up to 4% match.

• Perform an evaluation in the home and develop a care plan in collaboration with PTAs.
• Help patients with mobility and movement to regain independence.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure quality care.
• Evaluate and report patients’ progress to the medical director.

• Current unrestricted California Physical Therapist license.
• Current BLS/CPR certification.
• Valid California driver’s license.
• Knowledge of local community resources.
• Understanding of individual development and human behavior as it relates to illness/injury and cultural influences on health care.
• Knowledge of state and federal home‑care regulatory guidelines preferred.
• High‑acuity home‑health experience highly preferred.
• Experience working with an interdisciplinary team highly desired.
• Case management fundamentals highly desired.
• Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership skills, organized.
