All opportunities require field visits to patients in their homes.

• Conduct Start of Care (SOC) Admissions – OASIS
• Perform follow‑ups and evaluation visits as needed in the designated territory
• Make home visits to patients in the assigned territory
• Reconcile medication, provide wound care, and perform treatments and procedures
• Monitor client conditions and report changes to the clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Initiate and administer IV medication (antibiotics, diuretics) infusions safely and according to established protocols
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided

• Current and unrestricted California RN license
• Current BLS/CPR certification
• Valid California driver’s license
• Experience conducting OASIS SOC assessments (highly desired)
• High level of clinical knowledge in home health care (highly preferred)
• Experience with high‑acuity patients (plus)
• Experience working with an interdisciplinary team (highly desired)
• Fundamentals of case management (highly desired)
• Prior experience with IV medication infusions (highly preferred)
• Knowledge of initiating intravenous therapy (plus)
• Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership, organized
