Home Health Weekend Admission Nurse (RN) - Start of Care
Location: Sacramento, CA (District: Lincoln/Northern Sacramento).
Employment Type: Part‑time.

All opportunities require field visits to patients in their homes.
- Conduct Start of Care (SOC) Admissions – OASIS
- Perform follow‑ups and evaluation visits as needed in the designated territory
- Make home visits to patients in the assigned territory
- Reconcile medication, provide wound care, and perform treatments and procedures
- Monitor client conditions and report changes to the clinical team
- Collaborate with physicians and agency staff to deliver the best care possible
- Initiate and administer IV medication (antibiotics, diuretics) infusions safely and according to established protocols
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided
- Dedicated support for case managers to reduce administrative burden

- Current BLS/CPR certification
- Valid California driver’s license
- Experience conducting OASIS SOC assessments (highly desired)
- High level of clinical knowledge in home health care (highly preferred)
- Experience with high‑acuity patients (plus)
- Experience working with an interdisciplinary team (highly desired)
- Fundamentals of case management (highly desired)
- Prior experience with IV medication infusions (highly preferred)
- Knowledge of initiating intravenous therapy (plus)
- Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership, organized
