We are looking for a compassionate part-time LPN to provide person-centered care and mentor direct support staff working with individuals who have intellectual and developmental disabilities (IDD) and mental health diagnoses.
What You’ll Do:Partner with Registered Nurses (RNs) to develop and implement individual care plans.
Monitor vitals, track health progress, and securely administer prescribed medications.
Train, educate, and observe caregivers on basic nursing tasks under the Illinois Nurse Practice Act.
Provide calm, skilled support during medical emergencies and behavioral crisis de-escalations.

What You Bring:1–2 years of direct LPN experience preferred (experience with IDD/mental health is a plus).
Ability to work independently, document accurately, and lift up to 50 lbs (assisting transfers up to 250 lbs with mechanical/manual lift devices).
