Shift InformationShift structure: 12‑hour shifts. Full‑time positions: 30+ hours weekly; part‑time positions: up to 29 hours weekly.

Licensed Practical Nurse – Charge Nurse (LPN). The LPN Charge Nurse plans, organizes and assists in providing direct and indirect resident care following established policies and procedures. Orients, instructs and supervises personnel.
Responsibilities- Assess and implement interventions for resident care based on nursing standards and state, federal, and company requirements.
- Make daily rounds to see all residents to ascertain condition and comfort.
- Initiate procedures to protect residents from injury and infection.
- Assist the Director of Nursing (DON) and/or RN with developing, implementing, and evaluating plans of care, including spiritual, emotional, psychosocial, and functional factors.
- Encourage and assist with restorative care in self‑care and group activities.
- Monitor nursing personnel compliance with nursing policies and procedures and competency of clinical skills; evaluate nursing staff performance with direction from the DON.
- Communicate with the physician as indicated by resident needs or status and assure follow‑through; maintain hand‑off communications with oncoming shift regarding resident care needs.
- Oversee direct care staff and ensure staff complete tasks and cares as assigned.
- Speak, read and write English in an understandable manner.
- Apply principles of basic general math.
Licensed as an LPN with state licensure. Ability to work in a fast‑paced environment, supervise staff and coordinate care across shifts. Strong communication and math skills. Demonstrated commitment to compassionate resident care.
