Registered Nurse - Hospice - PRN

Vital Caring Group is looking for a passionate, compassionate registered nurse to provide hospice care in patients’ homes. The RN will deliver personalized, one‑on‑one care that supports patients’ independence and health goals while building meaningful relationships.

Schedule:

On‑call Monday – Friday 5 p.m.–8 a.m., Saturday and Sunday 24 h. Flexible schedule to support a healthy work/life balance. Transition from Duties of Traditions Health team members to employment effective January 1, 2026.

As a Registered Nurse, You Will
  • Evaluate and assess patients to determine appropriateness for care.
  • Utilize tools and resources to develop patient‑specific care plans with appropriate level and frequency of care.
  • Communicate with care team, physician, payors, patients and families to address care needs and fulfill care plans.
  • Deliver high‑quality skilled care and supervise other care team members.
  • Thoroughly document care delivery daily in EMR system.
  • Contribute to a culture of caring through individual accountability and teamwork.

  • Love learning, motivating, and inspiring patients to reach goals.
  • Enthusiastic about working to the highest level of the RN license.
  • Familiar and comfortable with technology; HCHB experience is a plus.
Experience to Deliver on Our Mission
  • Current RN license, valid state driver’s license, and reliable transportation.
  • Preferred:
    One year of experience as a Registered Nurse in an acute or clinical setting, with home‑health experience preferred.
