Job Title: PRN LPN (Licensed Practical Nurse)

Reports to: DON

Position Type: PRN Per Diem, work a minimum of 1 shift per week.

Shifts Available: 6:30a - 6:30p or 6:30p-6:30a

• Provide direct patient care under the supervision of licensed nurses and physicians
• Administer medications, treatments, and procedures as ordered by physicians
• Monitor and document patient vital signs, behavior, and progress
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Maintain accurate and up-to-date patient records
• Collaborate with other healthcare professionals to ensure seamless patient care
• Demonstrate a commitment to excellence in patient care and service

• High School diploma or equivalent required; LPN diploma or certification preferred
• 1 year of experience as an LPN; experience in long-term care or rehabilitation setting a plus
• Current LPN license in the state of Nebraska
• Ability to work variable shifts, including days, nights, and weekends
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Commitment to maintaining patient confidentiality and adhering to HIPAA regulations
