The Social Worker is the Coordinated Entry Specialist for Homeless Programs. This position enables VA homeless programs to fully collaborate with the community, including the Continuum of Care and community partners, in the coordinated entry efforts. The Social Worker supports the VA Homeless Program by providing continuity of care for homeless Veterans between VHA and community-based programs, as the Veteran transitions from homelessness into other programs, or other forms of community housing.
