MC Community Services is looking for an experienced case manager to join our Section 17 adult services team. We've just been approved to provide Community Integration Services across Penobscot and Piscataquis counties in Maine, and we need someone who knows how to connect people with the supports that keep them stable, independent, and engaged in their community.
This is not a desk job. You'll be out in the community working directly with adults living with serious and persistent mental illness — conducting assessments, building relationships, coordinating services, and making sure the supports around each person are actually working. You'll be the person who knows each client's full picture and makes sure nothing falls through the cracks. This is W-2 employment, with real supervision and a team behind you.
What the job looks like:
- Conducting biopsychosocial assessments and evaluating each person's needs
- Identifying and connecting clients to community services, natural supports, and resources
- Building strong, consistent relationships through assertive engagement
- Coordinating care across providers, housing, and community programs
- Monitoring whether services are working and adjusting when they're not
- Maintaining thorough documentation that supports continued care
What we're looking for:
- Active MHRT/C (Mental Health Rehabilitation Technician — Community) certification (required)
- Experience in case management, community integration, or similar community-based behavioral health work
- Comfort working independently in the field — homes, community settings, wherever your clients need you
- Strong organizational skills and attention to documentation
- A valid driver's license and reliable transportation
- Knowledge of community resources in the Penobscot/Piscataquis area is a strong plus

Active MHRT/C (Mental Health Rehabilitation Technician — Community) certification required. Experience in case management, community integration, or similar community-based behavioral health work. Strong organizational and documentation skills. Valid Maine driver's license and reliable transportation. Knowledge of Penobscot/Piscataquis community resources a strong plus.
