Job Summary

We are looking for an ambitious and forward‑thinking Advanced Clinical Practitioner to join our dynamic Primary Care Network. This is an exciting opportunity to shape and deliver high‑quality, autonomous clinical care across our PCN, making a real difference to patients with complex, undifferentiated and long‑term conditions.

Practising at Masters level (or equivalent), you will work across the four pillars of Advanced Practice Clinical Practice, Leadership & Management, Education and Research playing a pivotal role in transforming how care is delivered in the community.

You will be central to delivering key Network Contract DES priorities, including Enhanced Health in Care Homes (EHCH), anticipatory care and frailty services. Working proactively with multidisciplinary colleagues, you will help reduce avoidable hospital admissions, improve continuity of care and support patients to remain well in their own homes.

IMP Healthcare is a Primary Care Network (PCN) comprising nine GP practices working collaboratively to deliver high‑quality, integrated healthcare to a population of approximately 74,000 patients across North Lincolnshire and surrounding areas.

The PCN brings together general practice teams and a wide multidisciplinary workforce to provide proactive, patient‑centred care closer to home. Key areas of focus include Enhanced Health in Care Homes (EHCH), anticipatory care, frailty, long‑term condition management and improving access to primary care services.

Responsibilities

The Advanced Clinical Practitioner (ACP) will provide high‑quality, autonomous clinical care across the Primary Care Network (PCN), assessing, diagnosing, treating and managing patients with undifferentiated, complex and long‑term conditions. Working within an agreed scope of practice, the ACP will deliver both same‑day and planned care, including home visits and care home ward rounds, contributing to improved outcomes and reduced avoidable hospital admissions.

The post‑holder will support delivery of Network Contract DES priorities, particularly Enhanced Health in Care Homes (EHCH), anticipatory care and frailty services. This includes proactive identification and management of high‑risk patients, comprehensive assessment, personalised care planning and coordinated multidisciplinary working.

The ACP will demonstrate clinical expertise, contribute to leadership and service development, support education and supervision within the team, and participate in audit and quality improvement to ensure safe, effective and evidence‑based care across the PCN.

Key Responsibilities
  • Clinical Care – Independently assess, diagnose, plan, implement and evaluate care for patients presenting with undifferentiated and complex conditions.
  • Undertake comprehensive history taking and physical examinations.
  • Analyse clinical findings and investigation results to develop differential diagnoses.
  • Initiate appropriate treatment plans, referrals or admissions as required.
  • Prescribe medication as an Independent Non‑Medical Prescriber in accordance with national and local guidelines.
  • Conduct home visits, care home reviews and ward rounds as required.
