As a PCN Pharmacist, you'll work as part of an evolving primary care network, supporting medicines optimisation, improving prescribing safety and helping to deliver joined-up care across General Practice and Community Pharmacy. This is a role where you’ll grow your clinical confidence, develop your experience and contribute meaningfully to local healthcare delivery.

No two days are ever quite the same.
One day you might be carrying out structured medication reviews for patients managing long-term conditions, while the next you're responding to prescribing queries, supporting audits or working alongside colleagues to improve medicines use across the practice population.
Whatever the challenge, you'll apply your clinical knowledge, professional judgement and patient-focused approach to ensure safe, effective and personalised care.
You'll build strong working relationships across your network, collaborating with GP practices, community pharmacy teams and other healthcare professionals to help patients achieve the best outcomes from their medicines.

Behind every prescription is a person who needs the right support
Your work will combine patient-facing care, medicines optimisation and professional collaboration. As a PCN Pharmacist, you’ll:
• Prioritise patient care by supporting patients with one or multiple medical conditions where medicines optimisation is required
• Undertake clinical medication reviews, review monitoring needs and produce recommendations for senior clinical pharmacists, nurses and/or GPs on prescribing and ongoing care
• Work closely with GPs, community pharmacy teams and other healthcare professionals, answering medicines-related queries and supporting safe, effective prescribing
• Undertake clinical audits of prescribing in areas directed by GPs, feeding back results and implementing changes with the practice team
• Manage the repeat prescribing reauthorisation process by reviewing patient requests, monitoring review dates and flagging those needing further clinical review
• Support signposting and referrals to community pharmacy and other healthcare professionals where appropriate
• Help practices achieve Quality and Outcomes Framework indicators and support the development of services with medicinal components
• Maintain accurate documentation, ensuring interventions are recorded and coded appropriately
• Use the Electronic Prescription Service tracker correctly to support claims and manage processes effectively
• Train and support primary healthcare teams on therapeutics and medicines optimisation
• Ensure compliance with professional, legal and regulatory requirements
• Offer health screening services, including blood pressure monitoring, diabetes screening, flu vaccinations and other vaccine administration
