The Triage Nurse will provide triage assessment, clinical support, assistance and professional expertise to all health care professional referrers requiring assistance and support with admission avoidance and hospital discharges. The Service will be the access point for all community nursing referrals for the locality referrals professionals requiring health care. The triage nurse will provide clinical triage and support where appropriate, ranging from signposting the referral caller to the appropriate service, making a direct referral to another service, or offering immediate clinical advice or treatment.
The post holder is expected to represent the organisation in a professional and courteous manner at all times, providing advice and information using a high level of clinical understanding and professional judgement. To establish and maintain excellent communication links with all health and social care professionals. To work independently, exercising assessment and evaluation skills to take a clear and comprehensive referral for the community nursing teams in the locality, obtaining all relevant documentation as appropriate for referral.
Job Responsibilities- Work as a key member of the Care Coordination Centre to ensure professionals can make referrals through one single point of access.
- Provide high‑quality, clinical triage using knowledge, skills and critical thinking and professional judgement.
- Control and manage calls effectively by using skilled questioning, active listening and excellent verbal communication skills in a calm, professional manner.
- Work as a member of the nursing team in conjunction with other professionals to promote seamless patient care.
- Be prepared to work flexibly, in the interests of self‑development and exigencies of the service. Actively participate in the ongoing development of the role and support the development of protocols and processes within the service.
- Possess the ability to prioritise tasks, cases and situations appropriately to ensure effective delivery during periods of high demand.
- Liaise and communicate clinical information verbally and/or via technical links to healthcare partners to ensure continuity of care for the caller.
- Refer any issue or concern outside the scope of the post holder’s responsibility or professional registration promptly to the Operational Manager.
- Identify and report any case of suspected or confirmed abuse to an individual, in accordance with agreed guidance and procedure.
- Be personally accountable for professional and ethical actions and omissions in compliance with the NMC Code of Conduct.
- Current registration with NMC
- Registered Adult Nurse
- Evidence of continued professional development relevant to role
- Ability to demonstrate sound knowledge and application of up to date evidence‑based clinical practice
- Effective time management with the ability to meet competing deadlines
- Competent IT skills working across a variety of systems
- Flexibility with working hours to enable 7‑day working
