About the Role
We are seeking a motivated and compassionate Practice Nurse to join our friendly and supportive primary care team. This is an excellent opportunity for an experienced nurse or a nurse looking to further develop their career within a modern and patient-focused GP practice.
As a Practice Nurse, you will play a key role in delivering high-quality nursing care, promoting health and wellbeing, and supporting patients with both acute and long-term health conditions.
Key Responsibilities- Deliver high-quality nursing care to patients in accordance with practice policies and clinical guidelines.
- Manage and support patients with long-term conditions such as diabetes, asthma, COPD, and hypertension.
- Perform cervical screening, wound care, dressings, and immunisations.
- Undertake health promotion and disease prevention activities.
- Conduct NHS health checks and routine screening procedures.
- Provide travel health advice and vaccinations where appropriate.
- Administer vaccinations and injections in line with national guidance.
- Maintain accurate and contemporaneous patient records.
- Work collaboratively with GPs, healthcare assistants, pharmacists, and the wider multidisciplinary team.
- Participate in quality improvement initiatives and clinical audits.
- Registered Nurse with a valid NMC registration.
- Experience working within primary care, general practice, or a community nursing setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Strong clinical assessment and decision‑making skills.
- Commitment to delivering outstanding patient care.
- Chronic disease management qualifications or experience.
- Cervical cytology training.
- Immunisation and vaccination certification.
- Experience using clinical systems such as EMIS Web or Systm One.
- Knowledge of QOF and primary care pathways.
