About the Role

We deliver a family‑centred service that enables individuals, families, and communities to achieve their optimum physical, psychological, educational, and emotional wellbeing through a public health approach. We are excited to be involved in the Family Hub model, building on our partnership with Early Years teams, Early Help, Housing and Social Care. The role offers a genuine opportunity to make a positive impact on families, across clinics, nurseries and family homes, with a comprehensive induction and training package.

Continuous NHS Service is recognised and your NMC annual fees are paid by Lincolnshire County Council.

About You

You must be a Registered Children’s Nurse (or other registered nursing domain with children experience) and have a current registration with the NMC.

Main Duties of the Job

1. Post holder will receive clinical supervision and safeguarding supervision to support their own practice.

2. Attend a post-qualification learning development programme and obtain evidence of post qualifying and continuing professional development.

Job Responsibilities

Under the supervision of a Health Visitor or Senior Children’s and Young Peoples Nurse, effectively manage a defined caseload to ensure early intervention and reduce health inequalities. Deliver care programmes that promote health, safety and development for children and young people, including certain aspects of the Healthy Child Programme such as antenatal groups, health promotion and core contacts where training is completed.

Work with the children’s health service to develop and maintain relationships with families, stakeholders, and collaborators, supporting an integrated provision of services as part of the Council’s Early Help approach.

Liaise with early help, social care and other agencies to agree the best way to meet the needs of the child or young person and identify when additional support is required.

Deliver specific health interventions based on individual needs assessments to children on child protection plans, child in need, TAC and ESCO, acting in accordance with safeguarding policy. Attend child protection conferences, core groups and meetings as allocated.

Use specialist clinical skills to identify risks and needs in safeguarding children and adhere to agreed safeguarding policies of the Children’s Services Directorate and Safeguarding Children Partnership.

Act as lead professional where relevant for children receiving a health intervention/care programme.

Provide mentorship and community experience for pre‑registration students and 1‑to‑1 support and supervision for Family Health Workers, including providing appraisals.

Support managers in delivering operational objectives, plans and targets, reviewing and monitoring them to improve team and Directorate policy.

Participate in quality improvement and audits to maintain excellent standards.
