Job summary

Lincolnshire County Council invites applications for an Advanced Practitioner West Review Team role within the West Adult Community Wellbeing Team.

The position is a practice leadership role for experienced Social Workers, Occupational Therapists or Nurses. It involves managing complex cases, coaching and mentoring colleagues, and contributing to service development and quality improvement.

• Manage a varied and complex caseload, undertaking assessments, support planning, reviews, and risk management.
• Lead and support colleagues in delivering strengths‑based, person‑centred practice.
• Provide mentoring, coaching, and reflective supervision to team members.
• Support less experienced practitioners through co‑working, guidance, and case consultation.
• Take a lead role in complex safeguarding cases, ensuring that legislation, policies and procedures are appropriately applied.
• Promote person‑centred approaches that maximise independence, wellbeing and community connections.
• Contribute to service development, quality assurance activities and continuous improvement initiatives.
• Act as a Practice Educator, mentor Newly Qualified Social Workers and support student placements.
• Deliver workshops, group supervision and learning opportunities to develop practice across Adult Care.
• Build strong working relationships with health partners, voluntary organisations, communities and other agencies to deliver integrated support.
• Deputise for the Team Manager where required.

• Hold and manage a complex caseload with supervision and advice from the Lead Practitioner.
• Work with individuals, families, carers and communities to facilitate informed choices and decisions.
• Provide reflective supervision, coaching and mentoring to embed strengths‑based practice.
• Model positive responses to uncertainty and changing expectations.
• Demonstrate core leadership competencies such as Future Focused, Political and Commercial Astuteness, and Drive for Results.
• Ensure safeguarding policies are followed, promoting a personalised safeguarding model.
• Deputise for the Lead Practitioner and represent the team at multidisciplinary meetings.
• Co‑work specialist assessments with less experienced members to support development.
• Supervise or mentor students and act as a Practice Educator where appropriate.
• Research resources, identify gaps and contribute to statutory and national performance indicators.
• Implement Adult Care quality assurance systems and maintain compliance with Equal Opportunities policy.
• Use law, regulatory and statutory guidance to inform practice decisions.
• Deliver excellent customer service, supporting equality and diversity objectives.
• Stay updated on relevant legislation, procedures and professional codes of conduct.
• Champion organisational transformation, cost‑effective practice and self‑service where appropriate.
• Concentrate on specific areas of work as required by organisational needs.
• Recognise professional limitations and seek advice when necessary.
• Navigate criminal justice systems to ensure Care Act compliance within the Prisons Team.
• Seek professional second opinion on legal issues impacting service users.
• Produce evidence‑based, person‑centred plans and manage identified risks.
• Maintain understanding of adult behaviours including mental health, domestic abuse, substance misuse and disability.
• Maintain accurate case notes, reports and provide evidence in court when required.
• Deliver regular practice workshops, group supervision and complex case mapping.
• Support, coach and mentor team members and take on champion roles such as finance champion or S.117 champion.

• Recognised qualification in Social Work (Degree, PGDip or MA/MSc), Nursing or Occupational Therapy.
• Registered with Social Work England, HCPC or NMC with renewal in place.
• Post‑qualifying Practice Educator Level 1 and/or 2 or equivalent post‑qualifying specialist award.
• Knowledge of relevant legislation and statutory frameworks for adult care.
• Understanding of Health and Social Care organisational arrangements, including ICBs and NHS Trusts.
