Hospital Adult Care Team Lincoln County Hospital and Northern Lincolnshire and Goole Hospitals (NLAG) Salary including 15% Attraction & Retention Payment paid monthly (currently in place until March 2027 and subject to review)

37 hours per week - Monday to Thursday: 8.45am - 5.15pm;
Friday: 8.45am - 4.45pm. Weekend working is required on a rota basis, with time off in lieu provided.

What will you be doing?
  • Completing Care Act assessments and support planning.
  • Working alongside ward teams to support safe and timely discharges.
  • Supporting people to identify their strengths and maintain independence.
  • Leading or contributing to safeguarding enquiries.
  • Supporting Mental Capacity Act and Best Interest decision-making.
  • Working closely with families, carers and partner agencies.
  • Managing a varied and complex caseload.
  • Supporting and mentoring colleagues.
  • Helping develop students and newly qualified social workers.
  • Deputising for the Lead Practitioner when required.

About you
  • Registered with Social Work England.
  • An experienced practitioner with at least two years' post-qualification experience.
  • Confident working with complexity, risk and uncertainty.
  • Passionate about strengths-based practice.
  • A strong communicator who can build positive relationships with people, families and professionals.
  • Someone who enjoys learning and developing their skills.
  • Most importantly, you'll be someone who wants to make a difference.
