Reliable Home Care Providers, Inc. is seeking a licensed Occupational Therapist passionate about serving individuals in their homes. This part-time position is ideal for a clinician seeking to gradually build their caseload with the support of an experienced and responsive administrative team. The role offers scheduling flexibility and the opportunity to work with a diverse patient population.

Primary Responsibilities:

• Provide direct, skilled occupational therapy services in patients' homes
• Conduct evaluations, develop individualized care plans, and establish patient-centered goals
• Deliver therapeutic interventions aimed at improving functional independence
• Collaborate with physicians, case managers, and family caregivers to ensure continuity of care
• Complete timely, compliant, and accurate clinical documentation in accordance with regulatory standards

Requirements

Preferred Candidate:

• Licensed Occupational Therapist in the State of Illinois (required)
• At least 1 year of home health or community-based therapy experience preferred but not required.
• Familiarity with electronic medical record (EMR) systems (Well Sky Home Health)
• Strong clinical judgment, communication skills, and commitment to professional standards
• Compassionate, punctual, and able to work independently
• Must have a valid driver’s license, reliable transportation, and proof of auto insurance.
