You’ll provide one‑on‑one care to geriatric patients in their homes in and around Lincolnshire, helping them regain mobility, reduce pain, and improve their quality of life. - Deliver in‑home physical therapy services
- Evaluate patient mobility, strength, and function to develop treatment goals
- Implement effective rehabilitation and therapeutic exercise programs for
086 adult and geriatric patients - Educate patients and families on recovery plans and preventive care
- Maintain accurate and timely home health care documentation
