Job Summary:

Familyway Home Care is seeking a compassionate and experienced Per Diem Case Manager / Registered Nurse (RN) to join our team. The role involves coordinating patient care, ensuring compliance with regulations, and overseeing home health aides. The Case Manager/RN will serve as the primary contact for clients and families, developing and implementing personalized care plans.

Key Responsibilities:
  • Patient Care Coordination:
  • Assess, develop, and oversee individualized care plans for home care clients.
  • Conduct assessments to evaluate health status and needs.
  • Monitor and ensure quality of care provided by aides.
  • Educate patients and families on health conditions, medications, and self-care.
  • Compliance & Documentation:
  • Maintain accurate clinical documentation per guidelines.
  • Ensure services comply with Medicaid, Medicare, insurance, and Joint Commission standards.
  • Perform regular home visits to monitor progress and adherence.
  • Supervision & Staff Coordination:
  • Train, supervise, and evaluate home health aides.
  • Collaborate with healthcare professionals to coordinate care.
  • Act as liaison among agency, clients, families, and providers.
  • Quality Improvement &

    Risk Management:

  • Address patient safety concerns.
  • Participate in quality assurance and case conferences.
  • Assist with audits and inspections.
  • Qualifications & Requirements:
    • Active RN license in New Jersey.
    • At least 2 years of nursing experience, preferably in home health or case management.
    • Strong knowledge of healthcare regulations and insurance guidelines.
    • Experience in care planning, assessment, and interdisciplinary work.
    • Excellent communication and organizational skills.
    • Ability to work independently and manage multiple priorities.
    • Proficiency with electronic medical records (EMR).
    Preferred Qualifications:
    • Experience with geriatrics, post-surgical, or chronic disease management.
    • Knowledge of state and federal home health regulations.
    • BLS/CPR certification.
