Nurse Practitioner

Position: Nurse Practitioner at Ross Memorial Hospital
Become a vital part of Ross Memorial Hospital's Health First Program as a Nurse Practitioner in Lindsay, Ontario. This permanent full-time role focuses on chronic disease management and outpatient care delivery.

The Nurse Practitioner will function autonomously, providing comprehensive clinical care and education in the Health First Program. You will collaborate closely with the wound care team and engage in independent and cooperative decision-making processes. Our ideal candidate is experienced in advanced nursing practice with a strong foundation in chronic disease management.

Key Responsibilities:

• Provide direct clinical care to patients with chronic diseases

• Collaborate with the wound care team for effective treatment

• Educate clients and families about disease management

• Support outpatient program initiatives and development

• Work independently while engaging with multidisciplinary team members

Requirements:

• Current Extended Class RN registration in Ontario

• At least three years of advanced practice nursing experience

• Membership in the Nurse Practitioner Association of Ontario

• Certified Diabetes Educator qualification or commitment to achieve

• Proficiency in Microsoft Office applications
