Position: Chronic Disease Nurse Practitioner Role
Step into a career as a Chronic Disease Nurse Practitioner with Ross Memorial Hospital in Lindsay, Ontario. This full-time position is dedicated to improving patient care within the Health First Program.

As a Nurse Practitioner, you will bring your advanced nursing skills to the forefront, delivering clinical care and consultation for clients managing chronic diseases. This role also allows you to collaborate with the wound care team and contribute to outpatient initiatives. We seek a highly motivated individual with a passion for promoting patient health through comprehensive care strategies.

Key Responsibilities:

• Deliver patient-centered clinical care in chronic disease management

• Work with the interdisciplinary team to provide comprehensive support

• Facilitate patient education on living with chronic conditions

• Maintain and improve outpatient programs connected to Health First

• Document patient progress in adherence to best practices

Requirements:

• Current Extended Class RN registration in Ontario

• At least three years of relevant clinical experience

• Active membership in the Nurse Practitioner Association of Ontario

• Willingness to become a Certified Diabetes Educator

• Strong communication skills and proficiency in Microsoft Office
