The BSO Acute Care RN is accountable to the Manager of the Continuing Care Program at Ross Memorial Hospital in Lindsay, Ontario. This role emphasizes collaboration with cross-sector teams to support patients facing complex behavioral challenges. You will utilize your skills in behavioral health care to coach and educate both patients and care partners throughout the process, helping to create effective care plans and transitions.
Key Responsibilities:
• Provide consultation and support to acute care teams
• Conduct specialized assessments for patients
• Facilitate complex case discussions among stakeholders
• Develop care strategies for responsive behaviors
• Optimize resource allocation for patient support
Requirements:
• Valid nursing registration from the College of Nurses of Ontario
• Experience in geriatrics and behavioral health required
• Ability to prioritize and manage caseload effectively
• Excellent organizational and problem-solving skills
• Valid driver’s license and access to a vehicle
