Join a dedicated team at RMH in the Emergency Department, focusing on frail older adults at risk for adverse outcomes. This permanent role requires advancing comprehensive geriatric assessments and promoting safe discharge plans. The GEM RN collaborates with internal and external partners to enhance care and reduce hospital admissions.
Key Responsibilities:
• Conduct comprehensive geriatric assessments for older adults
• Coordinate and implement targeted care interventions
• Liaise with community partners and hospital services
• Support safe discharge planning and home first philosophy
• Educate staff and enhance geriatric capacity within the ED
Requirements:
• Current Registered Nurse in Ontario
• Bachelor of Science in Nursing required
• Minimum 3 years of geriatric or emergency nursing
• Advanced education in geriatric care is necessary
• Proven ability to collaborate with interdisciplinary teams
