Provide community nursing care with Para Med in Lindsay, Ontario, using patient assessments and care plans during scheduled home visits. Support patients of all ages.
As a Registered Nurse or Registered Practical Nurse, you will care for patients with differing complexity across various settings. You will help patients remain comfortable at home while building rapport and trust. The work includes individualized assessments, ongoing care-plan monitoring, and professional documentation.
Key Responsibilities:- Assess individualized patient needs using appropriate tools
- Execute and continuously monitor patient care plans
- Build rapport and trust through collaborative engagement
- Maintain and update professional patient records
- Travel to scheduled patient residences within designated areas
- Current CNO registration as an RN or RPN
- Good standing with the College of Nurses of Ontario
- Valid driver’s license and access to a vehicle
- Reliability and flexibility for scheduled community visits
- Required after-hours on-call coverage for patient safety
